Is financial assistance available for services?
Yes. Community Care Connections provides for private pay, private insurance, and Medicaid-approved clients. We are also an authorized Veterans Administration benefits provider. For those who meet eligibility guidelines, we are a Home and Community-Based Services (HCBS) provider.
What are the eligibility requirements for financial assistance?
To be Medicaid eligible for Home and Community-Based Services (HCBS), the client must meet the following eligibility guidelines:
- Be at least 65 years old, or have been determined physically disabled.
- Be in frail or poor health, or at risk for nursing center placement.
- Be assessed by a qualified Independent Case Manager.
- Have countable assets of $2000 or less – home and vehicle are exempt as well as prepaid funeral plans. If countable income is greater than the allowable amount, you may have to pay for a portion of the service.
The determination of eligibility and services for HCBS recipients is performed by your local Area Agency on Aging. Senior Care Act and Older Americans Act programs are funded in part by your local Area Agency on Aging and the Kansas Department of Aging.
