Home Care Services
Community Care Connections Services provides compassionate in-home care, available 24 hours a day, including weekends and holidays. Service times may range from a few hours once a week to several hours each day. Services are tailored to the individual’s needs and desires.
Home Care Attendant
A Home Care Attendant is an individual trained to assist individuals with daily activities they can no longer perform or which they may need assistance with.
The two levels of care we offer are:
Level 1 – Homemaker Services
These services consist of assistance with:
- Grooming
- Meal preparation
- Dressing
- Laundry
- Household cleaning
- Shopping
- Recreation
- Arranging transportation to medical appointments and errands
Level 2 – Personal Care Services
These services consist of assistance with:
- Bathing
- Range of motion
- Catheter care
- Mobility
- Transfer and lifting
- Ostomy care
- Vital signs
- Medication reminders
- Other personal care needs
Respite Care
Respite care offers rest for caregivers who are currently assisting with the care of a family member or friend. We can provide assistance so that persons may take time away from the duties of caregiving for their own personal needs.
Wellness Monitoring
We provide a registered nurse (RN) who will make routine visits to your home to assess and monitor any treatment program prescribed by your physician or case manager. Education and counseling are available for your health care needs.
